If you leave Japan permanently, move to another city, become enrolled in an employer's health insurance plan, or if your visa (period of stay) has expired, cancel your health insurance by submitting the forms for withdrawing from the National Health Insurance Plan within 14 days of the change.
If you are moving to another part of Japan, register your address change at the Alien Registration Section of the municipal office of your new residence within 14 days of moving.
If you are leaving Japan, hand in your alien registration card at the airport. Doing this will let the authorities know that you have officially left the country and will ensure that you are no longer recognized as a resident. This is important for receiving refunds on pension payments, etc.
Give at least one month's notice before moving out, and discuss the refund of deposits with your landlord/landlady. Arrange for the moving (contact a moving company if necessary).
Arrange to have the services discontinued and pay the charges (water, gas, electricity).
Inform NTT and your registered international telephone company 10 days before moving or leaving to arrange for telephone transferring procedures.
Notify the post office of your new address, so that your mail to the old address will be forwarded to you for a period of one year.
Notify the police station of your address change and revise your driver's license. For a revision of the content of your vehicle registration , contact the Tsuchiura Motor Vehicle Inspection and Registration Office if your car is over 660cc, or the Light Vehicle Inspection Society if it is under 660cc.
